Complex Disability Care Support Queensland

Providing High-Intensity Support
Everyone is different. While some with disabilities require more support, our team is skilled in assisting those with complex needs.
In-home care from a nurse might be the best fit. During our initial consultation, we’ll determine this together. We have nurses equipped to deliver clinical and other expert care.
Together, we’ll discuss and document your required support, including a home visit for an in-depth understanding. The assigned nurse or skilled disability support worker, will be trained in your management plan. You’ll then have peace of mind knowing your care is custom-fitted for you. For example, we can help with:

Read John’s Story
Sean’s mother, Liz, tells his story. They live in Werribee.
After hearing about CNHC from another family in 2018, I decided to give them a call when I needed extra support for my son Sean.
Sean is profoundly disabled and requires carers who are well-trained, patient, caring and fit in with the rest of the family.
We started off small with one shift a week for several weeks, but over the years that has now grown to multiple shifts 6 days a week to help support our son’s needs and goals.
In 2019 they were able to provide extra carers to help after his scoliosis operation when he could not attend school. They also supplied carers during the days we were in lockdown because of the pandemic and his day service was closed. CNHC was able to provide help during his recovery from his second surgery in early 2020, and when he was in hospital very sick in 2021.
Hayley, Sean’s coordinator is easy going, understanding, and knows how important it is to keep us informed of any changes. Hayley is very dedicated to making sure that Sean’s carers are suitable for what he requires and is in constant communication with me to ensure things are going smoothly.
Truly, the extra help and support from CNHC was a big factor that helped us survive the multiple lockdowns during the pandemic, don’t know where we would be without them.

Have A Question About Our Complex Care Services?
Here are some of the questions we are often asked. Everyone has unique needs, so please contact us with your questions.

What level of complex needs can you support?
We can support very complex needs in-home. As we have nurses on our team, as well as disability support workers, this gives a lot of flexibility in how we can support you. For hoisting, transfers and sleepovers, we have specific requirements. We will go through this with you, and assess the set up, during our onboarding process.

How do I get started with complex needs support?
Our team works together to provide you with your support services. First, we will discuss your needs, and conduct an in-home assessment on location. Once we have a management plan in place, your support workers will be trained in what’s needed.
All of that needs to be in place before we start, in order to make sure that we can support you appropriately and safely.

What set up do I need for a sleepover?
The Support Worker must have access to a separate room with a bed, use of appropriate facilities (including staff facilities where these exist) and free board and lodging for each night when the employee sleeps over.
The room where the Support Worker is to sleep must have a door that closes and a heating and air conditioning system available so that they can sleep comfortably.
The Participant must have an efficient way of waking up the sleepover Support Worker (e.g., buzzer, alarm system, etc.)
